VALUE-BASED CARE ACOs are reshaping post-acute care through preferred SNF networks ACOs are reshaping post-acute care. Accountable Care Organizations (ACOs) are becoming an increasingly important strategic partner for long‑term care (LTC) and skilled nursing facility (SNF) operators. For many facilities, participation offers a pathway to stabilize referral volume, reduce reimbursement risk, and gain access to clinical and analytical infrastructure that would be difficult to build independently. Research shows that ACO participation can strengthen care coordination and improve outcomes for medically complex populations. One of the most influential strategies emerging within Medicare ACOs is the development of preferred SNF networks. These networks are designed to manage post‑acute care quality and spending by aligning hospitals, ACOs, and SNFs around shared expectations, data transparency, and coordinated care pathways. When implemented well, they create opportunities for deeper operational and financial alignment across the continuum. Challenges LTC and SNFs face in today’s ACO environment Despite the promise of these partnerships, LTC and SNF operators face several persistent barriers: Limited internal analytics capabilities. Many facilities struggle to produce timely, credible data demonstrating their value on cost, quality, and outcomes—metrics ACOs increasingly expect from preferred partners. Few direct financial incentives. Most ACOs do not offer formal financial rewards for preferred SNFs, even as they set clear expectations around communication, length of stay, readmissions, and care coordination. A mismatch between current ACO models and facility‑based care. The American Health Care Association has urged CMS to develop an ACO model specifically tailored to nursing homes and assisted living communities, noting that existing models were designed primarily for individuals living in the community rather than in care facilities.⁶ These gaps make it difficult for SNFs to fully participate in value‑based care arrangements, even when they are eager to collaborate. Strengthening the role of SNFs through better documentation and insight To thrive in ACO partnerships, SNFs must be able to tell a clear, data‑driven story about their performance. This requires complete, organized, and clinically credible documentation—something many facilities struggle to maintain consistently. Excelas supports LTC and SNF operators by transforming medical records into structured, defensible insights that highlight quality, compliance, and clinical outcomes. By strengthening documentation and analytics, facilities can better demonstrate their value, meet ACO expectations, and position themselves as high‑performing partners within preferred networks.
