VALUE-BASED CARE Assisted living, value-based care share common goal of better outcomes for residents “For assisted living, value-based care creates incentives to keep residents healthy and manage chronic conditions, and [it] aligns financial incentives with better outcomes for residents,” said Walter Lin, MD, CEO and founder of Generation Clinical Partners, at the Post-Acute and Long-Term Care Medical Association’s (PALTmed) recent virtual symposium on the evolution of assisted living. The core concept of value-based care, he said, is provider accountability, which measures outcomes and ties payments to results. The model encourages preventive care and proactive management of chronic conditions to avoid costly interventions later. The model relies on data analytics to drive decision-making, he added. Lin said the goal of value-based care for assisted living residents is to keep them out of nursing homes. Treating chronic conditions more effectively and being accountable for outcomes, he said, makes clinical care providers better partners to assisted living administrators.
